Care Manager

As the Care Manager (Grade 5 RN), you will have experience in a Memory Support Unit and be responsible for ensuring our residents receive exceptional care by leading and maintaining high performing and culture-driven teams and be pivotal in maintaining positive stakeholder relationships that contribute to a safe and caring environment.

What You’ll Be Doing
 

• Collaborate to understand residents living with dementia in our Memory Support Unit with best-practice support and care
• Develop recommendation plans and behaviour support strategies
• Supervise care staff and oversee roster requirements
• Perform necessary Critical Incident Management and expert clinical support
• Support residents, families and visitors in a highly professional manner
• Ensure assessments, care plans and reviews are undertaken appropriately, on time and well documented
