As the successful applicant you will work independently to provide GEM and Rehab at Home patients with high quality, flexible assessment, responsive therapies and care in the patient's homes, it is a challenging role, expecting you to work both autonomously but also contribute to a dynamic, highly skilled multidisciplinary team for this 7 day service.
