About The Role
- Managing the Chronic and Complex Care Service, including heart failure, endocrine/diabetes, pain service, co-ordinated care outpatients and COPD Services.
- Actively contribute to the overall strategy and leadership within the clinical program, providing sound financial, administrative, operational and human resource management for all clinical services within the portfolio.
- Direction, co-ordination and supervision of staff within the Chronic and Complex Care team to ensure safe and efficient clinical practice.
- Proven ability to coordinate multidisciplinary teams, ensuring high-quality, patient care across a range of complex conditions and health care settings.
- Strong critical thinking, decision-making, and problem-solving skills, with the capacity to manage and prioritise.
About You
- Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA) or an Allied Health professional with relevant post graduate qualification in management and/or equivalent experience.
- Demonstrated high level leadership within the workplace; ability to plan, implement and evaluate local service delivery and to form productive working relationships.
- Highly developed communication, negotiation, facilitation and conflict resolution skills.
- Demonstrated experience managing operational, financial, human resource & workplace health and safety considerations.
- Understanding of the role and function of community health services, with ability to comprehend the navigation of a patient's journey across hospital, community and primary care services.
