Transition Care Case Manager

An attractive position is available for an experienced allied health or nursing professional to join our Transition Care team working as a case manager based at St George’s Health Service in Kew.

The Transition Care case manager assumes a key role in the co-ordination and case management of clients within the Transition Care Program and works as part of a multidisciplinary team.

Duties
 

• Direct provision of high quality and evidence-based patient centered care.
• Undertake holistic comprehensive assessments in collaboration with the client using case management principles and practice frameworks including strengths based approaches.
• Collaborative care planning with clients, carers and/or families in the context of their health, functional, psychosocial, cultural, environmental and economic needs.
• Facilitation of client generated goals that are realistic, time limited and based on addressing health and wellbeing needs
